Here's How AgeWise Can Help You or Your Loved One

Home Safety & Accessibility Evaluations

Every home tells a story, and every home has its challenges. Our comprehensive safety and accessibility assessments take a close look at your living space to identify potential hazards, barriers, and areas that may limit independence. We walk through each room together, evaluating everything from fall risks and lighting to bathroom safety and mobility concerns. At the end, you’ll receive clear, personalized recommendations and a practical plan to make your home safer, more comfortable, and better suited to your needs today and in the future.

General Therapy Services

Staying active and independent takes more than just a safe home. It takes strength, balance, and confidence in your daily routines. Through personalized therapy, we help you build the skills you need to manage everyday tasks, regain energy, and continue doing the things you love. Whether it’s improving mobility, boosting endurance, or making daily activities like dressing, cooking, or home management easier, we bring therapy right to your home and tailor it to your goals.

Caregiver Education & Support

Caring for a loved one comes with many challenges, but you don’t have to figure it out on your own. We provide hands-on training in safe mobility, transfer techniques, and daily routines, along with practical tips for managing your stress. With clear guidance and ongoing support, we help you feel prepared and confident in your caregiving role.

Fall Risk Assessments & Prevention

Falls don’t have to be part of aging. We’ll walk through your home together, spotting hidden risks and making simple changes that improve safety. Through personalized therapy to strengthen balance, mobility, and confidence, along with education and practical tips, we help reduce risks and support your independence.

Specialized Cognitive Assessments

Understanding how memory, thinking, and problem-solving skills affect daily life is an important part of planning for the future. Our cognitive assessments provide a clear picture of someone’s ability to safely manage everyday tasks and live independently. With results that are easy to understand, we guide families in making confident decisions about care, support, and next steps.

Home Modification Recommendations

Sometimes a few simple changes can transform your home into a safer, more comfortable place to live. We provide personalized recommendations, from rearranging furniture and reducing clutter to suggesting larger updates like ramps or bathroom modifications, always tailored to your needs and budget. Our goal is to help you stay independent and confident in the home you love.

Telehealth

Access expert therapy from the comfort of your home. Our secure telehealth sessions make it easy to receive personalized care, support, and guidance without the need for travel. They are perfect for follow-ups, consultations, or times when an in-person visit is not possible.

Chronic Condition Management

We provide specialized care for individuals managing chronic conditions such as arthritis, stroke, Parkinson's disease, Multiple Sclerosis, and other neurological disorders. Our therapy plans are designed to alleviate symptoms, improve function, and enhance your quality of life.

Equipment Recommendations & Training

Sometimes the smallest changes make the biggest difference. We’ll guide you in choosing simple, practical tools—like grab bars, shower chairs, or mobility aids—that fit your needs and help you feel safe at home. Just as important, we provide hands-on training and follow-up support so you and your loved ones feel confident using them every day.

Wheelchair Assessment & Training

Working closely with a DME provider, our evaluations determine the most suitable wheelchair options tailored to individual needs. We also provide personalized training after wheelchair delivery to ensure clients and/or caregivers can navigate their environments safely and confidently.

AgeWise Therapy Owner & Occupational Therapist

Your Trusted Partner in Safer, Independent Living

After 19 years as an Occupational Therapist, I’ve seen how the right support can transform daily life—for older adults and the families who care for them. WHAT YOU CAN EXPECT AS A CLIENT OF AGEWISE...

Meschelle E.

Shannon does an excellent job! Shannon shows a genuine concern for the patient and caregiver! Shannon shows us new strategies and techniques to make life easier for patient and caregiver! She excels in what she does and only wants what is BEST for patient and caregiver! We are truly blessed that she was our occupational therapist! I highly recommend her for she always gives 110%! We need more people like Shannon in the world that cares so deeply!

I cannot say enough about the difference having Shannon help has made for our peace of mind. My mother was starting to feel nervous about using the bathroom alone because of slippery floors and tight spaces. Shannon came out, assessed the bathroom, and pointed out safety concerns we hadn’t even considered. She gave us simple, practical solutions. What I appreciated most was how personalized the recommendations were. She took the time to explain why each change mattered, making us feel confident about the adjustments. Now, my mom feels safe and independent again, and I have peace of mind knowing she’s less likely to fall. If you want someone who sees the little things that make the biggest difference, I highly recommend!

Robert T.

When I first spoke to Shannon, I was nervous about losing my independence. Even simple tasks like showering or getting ready for the day left me drained and unsteady on my feet. I knew I needed to make some changes, but had no idea where to start. Shannon gave me strategies to build my strength and balance again. Over time, I noticed my energy coming back, and I wasn’t as fearful of falling anymore. The best part? She was able to walk me through real life scenarios I was struggling with in my own home. We even worked on getting on and off the floor safely so I even now have the confidence to get down on the floor and play with my grandkids again, something I thought I’d lost forever!
